We have a new web based system to meet your needs. You will no longer have to send a written request; the release is obtained in our applications from the doctor. We do not hand write and return any credentialing requests via mail or fax.

You can obtain the Confirmation of Coverage by accessing this link. If you have never used our system before please follow the instructions and create a new account. You will need your e-mail address, a password of your choice and your facility name and address. Please make note of your password for future use, If it is misplaced, you will need to create a new account under new user.

After logging in you will need the doctor's policy number and the last four digits of the doctor's social security number. You will then be able to print the Certificate of Insurance with claims history.
